1. This application form is to be used if you are already accredited and you are at the end of the 4 year accreditation period.
2. Cheque Payment of $550 for the main office and $300 for each branch office should be attached to the application

4. If there are any new forms or changes to forms that were audited previously, these should be enclosed with the application.
5. You must complete the audit within 1 month of the expiry. It is therefore advisable not to delay the re application as this may temporarily lead to a suspension of the licence and administration and or reapplication fees may be payable.
6. Please note that it can take up to 4 weeks for the appointment to be fixed for the audit.
*7. Late applications will have to pay a handling and late fee of $100 before the audit can be conducted. Late in this case means that the application is received after the one month set for completion of the audit. So if your audit is due in January, you must complete it by end February. Any application received after February will be subject to the late/handling charge. The onus is on the EA to ensure that this is complied with.
